It is very important that you notify the Practice if you have updated your personal contact details. We require your current address details so that we may contact you regarding your personal health.
If you do not inform the Practice of your correct address you may be removed from the list. Our local Health Board routinely asks GP Practices to check patient addresses to establish if they are still currently registered at the address provided by the patient. If your GP Practice is unable to confirm your details, then the Health Board may remove you from the Practice List.
