East Dunbartonshire Improving the Cancer Journey (ED ICJ) is a new service provided by East Dunbartonshire Health & Social Care Partnership (HSCP) in partnership with Macmillan Cancer Support. The ED ICJ service will offer people with a cancer diagnosis, access to the support they need in a timely manner.
We work with our local primary care colleagues, GP’s and Local Authority partners, who can send referrals on your behalf. Once we receive a referral, we will ask you to complete a Holistic Needs assessment (HNA) to highlight the most important issues to you. The HNA is a concerns checklist which includes all areas of concern; physical, emotional, social, financial, family, spiritual and practical. If anything listed on the form has caused you concern recently, please score the concern from 1- 10, with 10 being the highest.
We then offer a consultation with a dedicated Cancer Wellbeing Practitioner who will discuss your concerns so we can advise and support you.
Referring into the service is easy, via telephone, email or website. Patients can self-refer via these pathways. We also welcome referrals for family members and carers of a person affected by cancer.
